Kindly email the scanned copy of the four (4) required documents to sog@cdcholdingsinc.com with the following email subject format, ‘CTS – Last Name, First Name Middle Initial / Unit Number’

Example: CTS – Dela Cruz, Juan A/ HT-1234

Note: *E-Signatures are also accepted
